According to sf.ruggedelegance.com, Tartine will do custom cakes with 24 hour notice. If that's true, you can't go wrong. All their work is beautiful and delicious.

I would suggest Citizen Cake. I was out there last October, and ended up having four desserts when I stopped there. I'm not sure on prices for whole cakes and stuff, but I imagine that it's worth it. They do incredible stuff.

I second Citizen Cake in San Fran..off Van Ness..had a beautiful bday cake made there for my boyfriend...
