Food Network had a one hour special about this sort of thing. One of the longer segments was devoted to a company based in south SF that did it.

Sorry -- no idea what the show or the company was named. I seem to recall there were "cook your own" meals (with prepared ingredients and explicit instructions) for about $50, and "we cook in your home" for about $150.

Jane Peal (www.pealciusine.com) delivers gourmet vegetarian meals; check the web site for menus and dates. I have not yet tried her stuff personally, but have heard good things, and she's been in business for quite some time.

We used to have a great service called CookExpress, where they'd deliver full meals in their component pieces, to be finished (using idiot-proof instructions) at home. They went under back in '99 or '00. I've heard rumors that there's a new one called Home Express or some such, but I can't seem to find it. Anyone else?

Are any of those places that used to deliver meals from a lot of local restaurants still in business-you know, the ones that have all the menus in them (prices raised of course?)
